We require the following documents below for all services

18 and Over
Clients who are 18 years or older must provide their ID for any service, including removing or changing jewelry. We need your ID every time you want any procedure, even if you have come in before. For our documentation, we must have a valid,
non-expired ID.
It must be the original; photocopies or on phone are not valid.
If you have any questions about acceptable IDs, please give us a call before you make the trip. We don’t like turning away clients, so we’ll do our best to work with what you have to help you meet our ID requirements.
Accepted Forms of ID's
Minor (17yrs - 13yrs old)
For any piercing or jewelry insertion service for a minor, we will need three documents. The minor will need a valid government-issued document that shows their date of birth as well as a photo ID. A driver’s license, state ID or passport, and original state birth certificate* will be needed. Additionally, we will need a valid (non-expired) ID from the parent or legal guardian. We will verify with driver’s license with any government-issued photo ID with a name that appears on the minor’s birth certificate or guardianship paperwork.
Most importantly, all documents must be original or notarized copies of the original.
Due to extensive healing and possible scarring, some piercings are not offered to minors, please call us for information
Accepted Forms of ID's for Minors 17yrs - 13yrs old
Minor (12yrs - 6yrs old)
For ear piercings or jewelry insertion services for a minor, we will need three documents. The minor will need a valid government-issued document that shows their date of birth as well as a photo ID and original long form state birth certificate. If the minor does not have a government issues ID we are happy to email our Notarized Letter. Additionally, we will need a valid (non-expired) ID from the parent or legal guardian that proves they have legal responsibility for the minor. This could include a driver’s license with matching last names to the minor’s ID, or any government-issued photo ID with a name that appears on the minor’s birth certificate or guardianship paperwork.
Most importantly, all documents must be original, they can not be copies.
We only offer earlobes piercings for this age group by appointment only
Accepted Forms of ID's for Minor 12yrs - 6yrs old
Minor (5yrs old - Newborn)
For ear piercings or jewelry insertion services for a minor, we will need two documents. We would need an original long from state birth certificate (not a hospital certificate) and if the minor has a passport, we can accept it as well. Additionally, we will need a valid (non-expired) ID from the parent or legal guardian that proves they have legal responsibility for the minor. This could include a government-issued photo ID with a name that appears on the minor’s birth certificate or guardianship paperwork.
Most importantly, all documents must be original, they can not be copies.
